Before we consider your home for a rescue Great Dane we MUST receive:

A completed Application
Your $25, non-refundable, Application Donation
If you live in the areas listed below, please contact UMGDR at contact@thegreatdanerescue.com to ensure a volunteer is able to conduct your home visit.

Once we receive your Application AND Application Fee, we will schedule a home visit before approving your application and placing you on the waiting list for a dog

UPPER MIDWEST GREAT DANE RESCUE

Please fill out this application fully. This application is to help us determine if the adoption is in the animal’s best interest, and to assist the potential adopter in finding a Dane compatible with their lifestyle. Understand that my application can be denied without a reason given. We hope you will agree that the animal’s welfare be our prime
concern.

In order to be considered for an adoption you must:

Be at least 18 years of age.
Have the knowledge and consent of all adults in your home.
Have landlord consent if applicable.
Although we would prefer to adopt to families with children over 8, we will still accept applications from families with younger children. If you have children under 8 the application approval process will be more in depth.
Pay the non refundable $25 application fee
Pass a criminal background check (we are looking for animal cruelty/neglect charges).
Have two positive personal references
Need to have a positive reference from current veterinarian.
Home visit must be completed.

All completed Adoption Applications, and Home Visit Evaluations are confidential property of UMGDR. These completed forms are shown to volunteers on an as needed basis.

At no time, unless it is court ordered, does UMGDR share this information with another organization, inquiring individual, or the applicant.

I affirm that I have never been charged with cruelty to animals or any other animal related charges
I affirm that I have never been charged with domestic abuse or crimes of violence.
I understand that UMGDR reserves the right to deny my adoption application for any reason whatsoever including, without limitation
if UMGDR finds that such adoption would be contrary the UMGDR”s adoption policies, in violation of any state or local ordinances,
or not in the best interest of the adoptee, in each case, as determined by UMGDR in its sole discretion and in the event that my
application is denied, UMGDR shall not be required to disclose the reasons for such denial
I understand that failure to comply with the completed adoption contract could result in my inability to adopt other dogs from
UMGDR and could require that I return the Adoptee to UMGDR upon demand
I understand that UMGDR cannot be held liable for any actions of the dog once I adopt it.
